I recommended a few years ago that the IRC dispute resolution process
should involve non-ops. 

The "wishy-washy" problem that is at issue is cultural. It's the
accumulation of what really are minor incidents of injustice or abuses
of power that are being described here. For example, some ops have the
policy of never questioning each other. Apportioning blame to
individuals for minor incidents, though, won't solve anything. There
need to be fundamental changes in how you do things.
