I have just experienced this stuff.

After searching for 10 minutes I stumbled upon this bug.

Deleting formhistory.sqlite fixed everything. I have corrupt
formhistory.sqlite lying in safe folder; as it might contain personal
info I don't feel like attaching it here. If you still need it, contact
me.

As I understand, it is not really ubuntu issue, but firefox, right?
