What steps did you take to reproduce it, John ?

I already posted in comment 7 that it takes 3 steps and is 100% reproducible. Here it is again:
 - boot the desktop CD
 - connect to internet
 - open the URL https://launchpad.net/codeofconduct in Firefox

The fact that you already signed it is irrelevant, unless you did not
follow the above steps (and you logged into launchpad).

I can create a video if you have trouble following the above steps.
