we need a step by step instrcutions on how to reproduce this reliably;
preferably with a website that doesnt need a login.

Anyway, I think its an extension as you have a bunch installed. Disable
them or uninstall them to get a cure. I you still get it even though
everything is disabled, reopen this bug. Thanks!
