Openstack problem

Hi,
I have problem without install new single openstack server. 1 week  ago I
can install this without problem but now when run openstack-install I have

In my case is worst because I try  install Ubuntu on utopic (no luck this
same problem) and try install patched  kernel 3.18 ( my server won't  boot
problem with  keyboard). Maybe someone have solution?
