Sergio Costas has proposed merging ~rastersoft-gmail/livecd-rootfs:migrate-core-desktop-as-core-subproject into livecd-rootfs:ubuntu/master.

Commit message:
This patch updates Core Desktop to version 24, and also makes
it a subproject of Core, so it shares nearly the whole project
files and simplifies maintenance. It is based on a proposal from
Michael Hudson-Doyle.

This PR overrides https://code.launchpad.net/~rastersoft-gmail/livecd-rootfs/+git/livecd-rootfs/+merge/472458 by implementing the idea from Michael Hudson-Doyle of transforming Core Desktop into a Core Installer subproject. This greatly simplifies the maintenance, since nearly everything is shared with Core Intaller. At the moment it doesn't exist a way of using `snap know' to get the .model file, so it is currently being shipped in the repository, but the idea is to have it in the future, so the 05-prepare-image.binary hook would be even more generic.
