Looking at these processes, most of them refer to instances that no
longer exist:

There are no obvious errors in the logs to suggest anything went wrong.

This is version 1:2013.2.1-0ubuntu1~cloud0 running on precise.

Bug #1250231 appears to be describing something very similar if it helps
in any way.
