I updated the bug some, broke your patch out to an attachment, and
verified that the fix exists upstream.

Concerning your iscsi patch, I suggest you send it to linux-scsi
for further review. I suspect they'll ask that the split-brain
avoidance be broken out under the context of a new parameter, configurable
at module load and runtime. Running scripts/checkpatch.pl before sending it
would be prudent.
