I am also having issues with empathy and SIP calls. 
I am using Karmic Koala with current updates, Empathy 2.28.1.1, telepathy-sofiasip 0.5.18-1, and libsofia-sip-ua0 1.12.10-4
I am using a sipgate.de account.
Here is what I saw ...

1. The SIP REGISTER request does not have an EXPIRE header, unless it is a de-registration, which carries "Expires: 0"
I assume if the header is missing, some default value is applied.
To my current knowledge it would also be possible to add an expires=123 parameter to the CONTACT header.
Well, the registration procedure currently works in my setup.

2. When opening a call with INVITE, I get a 401 UNAUTHORIZED response. Empathy sends another INVITE request, but it does not add the AUTHORIZATION header. This goes on for a while ... 
The call is never set up.

If I can provide extra information, please let me know!
