I doubt your config is causing the slowdown.  Keep in mind that there
is a variable that is always changing in your setup, the data being
scanned.  There have been instances when clamd slows to a crawl while
scanning various types of data such as a file with long runs of zeros
(this particular bug is fixed, but I'm using it as an example).  If
you have the resources, once you locate what is slowing it down, copy
that data to a fresh install with clamav in a virtualbox vm (or
whatever vm software you use).  If it still slows down there, you may
want to search for similar problems on clamav's mailing lists and bug
tracker (http://www.clamav.net/lang/en/support/).
