Those symptoms tend to point to hardware issues, particularly the i/o
reject regions. Does the device node still show up at /dev/sdb ? If the
drive's connecting cable remains snug, a reboot of the OS should at least
show the device being probed in syslog, and then you may be able to glean
some information via smartctl if the device is capable (not necessary to be
mounted).
