hmmm, no one answered so I'll answer my way :)
please note that there should be a much user-friendly (GUI) way to do
this in ubuntu, but I don't use it & I'm not even aware of it...
basically: use "wvdial"
You can read about how to use it here:

start where it says "Configuring for the First Time" because it comes
with ubuntu by default and you won't have to compile it or anything.
although reading that text would be much nicer, here is a quick
review:
in the terminal:
sudo wvdialconf /etc/wvdial.conf
this will setup wvdial configuration (note: if this cannot find a
modem, you may have a "winmodem", and you may need to use "scanmodem"
-good luck :( )
sudo gedit /etc/wvdial.conf
input your username and password and phone where it asks then close.
sudo wvdial
this will dial up and connect.
if it fails, try putting this to the end of wvdial.conf
Stupid Mode = yes
if fails again, try adding this as well and experiment
Carrier Check = no
if fails, paste the output of wvdial, someone may be able to help...
