Logitech Stereo USB Headset 250?

I can replicate this behaviour with a plantronics USB headset which
would suggest a generic bug with USB audio devices.  Sometimes it will
output sound (usually if you have it plugged in on boot) but will only
work at full volume.  Changing the volume level in the OS has no
effect.

Flaky performance shows up as well, as i tried using with Skype and
the thing just cut out for no reason.
