Get a joystick, break it in parts and put the buttuns at custom places.
Then program it via SDL joystick api.

For hardware design with input and output. Look at parport device.

Though you are not specific about what you want to do. Chances are there 
are pre made hardware and standard designs and library/drivers for what 
you want. That is impossible to tell because you just talk about very 
low level stuffs.
