I had it happen once recently - I presume a game I was playing had a memory
leak, because it's never happened at any other time.  I
have /etc/acpi/hibernate.sh hooked to my power button, and it would start
suspending, swap like crazy for a minute or so, and then everything would
be right back where it was.  It took me three tries before I saw the
message that it just couldn't fit the memory image on swap.
