Not sure about the LTS server, but you could try:

sudo apt-cdrom add
[this will add the cd to your sources list]
sudo apt-get update
sudo apt-get upgrade
sudo apt-get dist-upgrade

then after the upgrade, just to be sure:

sudo apt-get update
sudo apt-get dist-upgrade
sudo apt-get -f install
sudo dpkg --configure -a

[disclaimer; I've not tried this, so it's best if you do it on a test
machine first]

If you're still getting hash errors on your downloads try one of the
other mirrors:
http://www.ubuntu.com/getubuntu/downloadmirrors
And for upgrade mirrors, get to a desktop and go to
system->settings->software sources, "Other" and then click the same
"Select Best Server". That will ping all of the existing servers and
select the fastest one in response to the ping. Sometimes this works,
sometimes not. If you find a good one then use that for the rest of your
machines. Here is the list that you find in "Other":
