Do you have more than one video device? perhaps your webcam, and a TVcard.

I have a TV card, and a webcam, and udev, that loads all these devices 
randomly, oftentimes mixes up the order of these devices . Sometimes my TV 
card, which should be /dev/video0, is loaded as /dev/video1, and the webcam, 
which is /dev/video1 does not work, as the TVcard now has /dev/video1.

This is just a thought, but if you have more than one video device, it may be 
worth trying /dev/video1 for your webcam.

As i say this just a thought, as I have had similar problems with multiple 
video devices.
