Is eth0 mentioned in /etc/network/interfaces? If it is, network manager
will ignore it.

Attempting to plumb the interface manually might show up a few clues:

   ifconfig eth0 up
   ip add addr 192.168.99.9/24 dev eth0

... that sort of thing (not tested).
