kernel upgrade held back

Typically the kernel is not automatically installed since using it requires
a reboot. For desktops that's not a big problem, but for servers it's
useful to pick and choose what updated kernel to install and boot. To
install the kernels use 'sudo apt-get dist-upgrade'.
