Various possible reasons, such as multiple partitions, an NTFS volume,
an encrypted volume, an incorrect partition table.

Install GParted, and being very careful that you do NOT reformat your
hard disk instead, remove any existing partitions on the USB drive,
make a new MBR partition table and a new FAT32 partition. That should
work.
