This I see fairly (too) often, caused by old drive that didn't spin up (from a 
full shut down) as fast as the OS (Win & various Linux's) expected, many times 
a reboot will work, I've only jumped in part way though this thread so if I 
miss your point sorry

Also I've hit this by
1) an interupted "full-upgrade" due to a power-out
2) clicked blindly on a dialogue for an "updated Grub ;-<)", that made my boot 
drive "un-bootable" 

Bootable & Mountable/Readable are not the same thing
