Maybe OpenSBC [1]?

Asterisk [2] seems to be the Swiss Army knife of open source
telephony, so it may be able to do what you want, but I haven't
personally looked into it. I know some of the local CLUG (Cape Linux
Users Group) members use it quite a lot.
