* Always use the `unix_prefix' scheme for setup.py install in a virtualenv

* Revert inverting the supported/unsupported check for rtinstall.

* Py_DECREF: Add `do { ... } while (0)' to avoid compiler warnings.
* Fix build failure building the documentation.
