[Bug 369389] [NEW] Upgrade from Ubuntu Server 8.10 to 9.04 doesn't perform database upgrade?

Binary package hint: phpbb3

I upgraded from 8.10 to 9.04; prior to the update phpbb3 was working as
you would expect; after the update I am just getting an error message
when trying to access any page.

I performed the update by issuing the following commands:
sudo apt-get update
sudo apt-get upgrade
sudo apt-get install update-manager-core
sudo do-release-upgrade

Everything appeared to go smoothly - after the update process restarted the system, I was greeted with the following error when trying to access phpbb3:
"General Error
SQL ERROR [ mysql4 ]

Unknown column 't.template_inherits_id' in 'field list' [1054]

An SQL error occurred while fetching this page. Please contact the Board
Administrator if this problem persists."

Googling around, this appears to be the result of a mismatch between
database version and the PHP code - I issued a updatedb and locate to
find the PHP file (database_upgrade.php) being referred to without any
luck.  The upgrade instructions that are part of the Debian package are
woefully empty and provide no clue where to start.  How do I fix this?
