I  went to "Wine Configuration", Audio tab.  Unticked all sound drivers, leaving only EsounD Driver ticked.
I can confirm that sound playback is now correct - Thanks for the tip edalb!

This setting conflicts with the advice on the Spotify webpage
http://www.spotify.com/en/help/faq/wine.  I have emailed them about this
in case they want to update their webpage.

I'm happy - I have a successful work-around.
