[Bug 1090815] [NEW] Installer Crashed

Public bug reported:

I tried to reinstall Ubuntu 11.04 64-bit AMD from CD, which I burned.
The installer crashed, giving me a note to file a bug report here.

I was past the partitioning phase and the installer said it was removing
conflicting files (no doubt from my previous install which was borked).

I did not tell the installer to format any of the partitions; perhaps
this is the cause of the problem.

I have combined the requested log files into one attachment, which is
all that is allowed here.
