[Bug 995110] [NEW] install of 12.04 failed to restart, was trying to erase and reinstall

When installing 12.04, system failed to restart.
Manually rebooted and just get a blank screen.
Tried recovery mode, system hangs with meaningless (to me) messages.
Loaded previous version (11.10) and was trying to erase 12.04 and reinstall (system seemed to think it was installed?). Installer crashed just as I was about to choose timezone.
