I have just begun seeing this on my Ubuntu 13.04 Beta 2 system.  It
started appearing in/around initial disk fsck/mounting and renders my
system unusable, with messages repeating nearly every second.  Likely in
pairs every second or so much the same as the original reporter.

I believe in my case, it is the same pci: id every time, and usually a
few reboot attemps (hard power) and I get by without incident.

It only appeared in the last week or so, so I suspect exists in both
linux-image-3.8.0-18-generic and linux-image-3.8.0-19-generic

I have not been able confirm the specific device using udevadm (assuming
I need to provide the pci: id as an input) but plan to take a camera
shot on a future incident to identify the specific hardware, at which
point a hardware specific bug may be more appropriate.

  This started after i upgraded to 12.04. Got a new kernel this morning, upgraded and rebootet but still the same.
