[Bug 1605350] [NEW] apport info dialog about application crash doesn't contain application name

It's possible that `apport` displays a dialog which informs about the
crash of an application which doesn't contain the name of the
application which makes the dialog almost useless in comparison to a
dialog which would contain the name (see screenshot for details).
