SQUASHFS error: squashfs_read_data failed to read block 0x1304064b

It would appear that the image (ISO file) you downloaded could be
corrupt. Could you please visit
https://help.ubuntu.com/community/HowToMD5SUM follow the instructions
and report back as to whether or not the MD5SUM verified? Thanks in
advance.

Installer has also Check disc for defects option. You might find that
even easier.

  Crash in install

Status in ubiquity package in Ubuntu:
  New

Bug description:
  On the screen where partitions are entered, I forgot to select
  "format" the partition that already had Ubuntu 18.04 LTS. The install
  alerted and I returned to select. After that, an error message popped
  up saying I had a problem with the HD / CD.
