filing bugs against acpi-support for hotkeys

Hi everyone
I have got  list of not functioning hot keys (fn+key) with their key
codes and i want to file a bug against them.
Do i go and file a  bug for each one, or just one bug report with a list
of not working keys?
Also, what is the important stuff to include in the bug report? I'm
guessing th following are needed:
kernel
ubuntu version
hot key +what its supposed to do
key code
laptop model/manufacturer etc
is that it?
