[Ubuntu-SG] Sign up sheet/questionnaire?

Hello everyone,

Should we create a sign up sheet for today's workshop? Interested
participants could supply name & email and choose to be contacted by
TUSG, linuxNUS and/or SoC Computing Club? As well, we could ask them if
they are willing to be contacted to participate in a yet-to-be created
post-workshop online questionnaire that we could run off of Google Docs
or ubuntu.sg website.

I have attached an example.
