It's called 'bookmarks' in PDF lingo.

If you are asking about a tool available in Ubuntu, then it's pdftk, for
example. There probably are more. From pdftk ma page:

pdftk in.pdf dump_data output report.txt

Both PDF specification and evince, pdftk sources are open, so you are
welcome to take a look at how it is done, if this is what your inquiry
was about.
