CRTs use a partly analogue connection. A KVM is no problem.

LCDs and other flatscreens use an all-digital connection. The PC needs
to listen to data coming from the screen to know what resolutions the
screen can handle. Some of these data can't go over analog VGA cables,
and worse still, some KVM switches block the data -- the KVM does not
transmit it.

This stuff cannot be relied upon to work through a KVM.

Ditch the KVM and ditch the analogue SVGA cables. Use HDMI and I
predict all the problems will go away.

I think that all the other troubleshooting I have seen on the list is
totally irrelevant and it's missing the core problem
